This may happen when you are member of more than one team and that each team has defined a questionnaire with slightly different questions.

Currently the system suppresses duplicate questions if they are exactly the same.

For example, if team A asks "How is your motivation to train today?" and and team B asks
"How is your motivation to train today", both questions will be displayed as the question mark "?" is in one question and not the other.

To make sure that questions that are common to two teams are not duplicated, make sure that they are written exactly the same.

For example, if team A asks "How is your motivation to train today?" and and team B asks "How is your motivation to train today?", the question will be asked only once in the athlete's questionnaire.
