Can I add custom questions to the OSTRC questionnaire?

Yes. This can be done in 4 simple steps.

1. Open the Team Settings-Injury Surveillance (OSTRC) section 
2. Set the Custom Questions Page option to Show 
3. Add your custom questions, related answers and their respective format (all that apply, freeform text, fixed-length text, number, date and time)
4. Once done, click Save

Athletes will be able to answer these questions when completing their OSTRC questionnaire, on their app or via the website.

To export answers to the custom questions, select the Log & Stats-Injury Surveillance (OSTRC) menu options and open the Log tab. Answers will automatically be added on the Log tab.
