To add electronic signatures to your questionnaires/forms, open an existing or new questionnaire, add a question, name it 'Electronic signature' or something similar and select the Signature option.

Once done, assign the questionnaire and athletes will be able to draw their signature when completing the form.

PDF export will displays the complete signature.

Raw data export shows the signature status in column U
