Attaching multiple health forms / questionnaires to health problems

It is possible to attach multiple health forms to speficific health problems. Here is how to do this:

Step 1

Make sure health questionnaires are available in your system. This process is explained here: https://support.en.athletemonitoring.com/support/solutions/articles/13000025294-adding-a-new-survey-questionnaire

Step 2

Open Team settings-Health-Health forms and select/deselect the forms you'd like to use

Step 3

Open health history, click on an health problem and select the Forms tab. Then select the form, complete it and save.
